READ THIS AGREEMENT CAREFULLY
This Express Assumption of Risk, Complete Waiver and Agreement Not to Sue and Indemnity Agreement, is made by ____________________ ("Camper") if Camper is 18 years of age or older for himself, his agents, partners, associates, successors, heirs, assigns, representatives, attorneys, and all persons acting by, through, under, or in concert with Camper (and is made by the undersigned parent or legal guardian ("Parent(s)") of Camper for and on behalf of Camper if Camper is not 18 years of age or older) in favor of San Diego Christian Foundation, Inc., its past and present members, volunteers, agents, partners, associates, affiliates, directors, officers, employees, predecessors, successors, heirs, assigns, representatives, attorneys, and all persons acting by, through, under, or in concert with it (collectively, "SDCF"). Camper wishes to (and if under 18 years of age also the Parent(s) authorize and grant permission to allow Camper to) participate in the Mt. Palomar Bible Camp ("Bible Camp") to be provided by SDCF. In consideration for being accepted by SDCF for participation in Bible Camp to be provided by SDCF, the undersigned Camper and Parent(s):
Camper and Parents understand that SDCF from time to time produces promotional material relating to its programs and that Camper may be included in videotapes or photographs taken during Bible Camp so, without reservation or limitations, hereby assign, transfer and grant to SDCF, its successors and assigns, licensees, sponsors and all other commercial exhibitors the exclusive right to photograph and/or videotape Camper and to utilize such videotapes and photographs and Camper's name, face, likeness, voice and appearance as part of Bible Camp, in advertising and promoting similar future events.
I understand that SDCF is relying on this Agreement to accept Camper for participation in Bible Camp and would not accept Camper in Bible Camp if Camper did not execute this Agreement. We have read and understand this Agreement. We understand that by signing this Agreement, we surrender valuable rights. We do so freely and voluntarily. We further certify that we have received and read a copy of this Agreement.
Our goal is to provide a safe Christian environment for all campers. Below is a list of rules that we must enforce to protect our campers and maintain our high standards. Parents - please review these rules with your child. Parents and campers should sign below, indicating the camper's commitment to live by these rules while at Mt. Palomar Bible Camp.
I agree to behave in a manner that is both respectful of Mt. Palomar Bible Camp staff and campers, as well as the Palomar Christian Conference Center staff and facilities. I will comply with the rules of Mt. Palomar Bible Camp as outlined above. I understand that violation of these rules may result in disciplinary action, including the possibility of being sent home. IMPORTANT NOTE: If a camper must be sent home, the parents agree to come and pick him/her up without a refund.
It is wonderful to know that your son/daughter has chosen to participate in our annual Palomar Mountain Christian Camp. Our prayer is that your child will draw closer to our Lord and the cross of Calvary by something that is said or done during camp. We are very excited about this year's camping experience for your camper(s).
Each summer, young people come to camp and make a decision to follow Christ. The death, burial, and resurrection of Jesus Christ is the essence of the gospel message. God wants everyone to respond to this grace through faith (I Timothy 2:4-5; Ephesians 2:8-10). We pray the week your child is in camp that the Lord will use us to teach the message of God's love, mercy and grace.
This letter is our way of emphasizing the importance of baptism in a young person's life and making sure that you as parents/guardians are part of that decision. We will make you aware of your child's decision prior to any immersion. Time will be provided for questions, study, and visits with you and your child to ensure they understand the purpose of baptism and the ramifications of dying to self. Please provide an accurate recall number (home, cell, work) and any appropriate comments/instructions regarding the possibility of their conversion to Jesus Christ during camp. If you would rather them wait until they return home for more instruction, teaching, etc, please specify in the comments.
