Thank you for your interest in being a counselor at camp. Every year at camp, we work very hard to create an environment where our Lord and Savior can produce profound changes in the lives of those who attend and this year is no different. If you have joined us at camp in the past, you will notice a significant change in the application forms and the process due to new laws from the State of California.
Completing these forms is a requirement for service, and because of the nature of this being a ministry to youth and children, the camp directors reserve the right to turn down requests to serve.
As the camp's directors, we believe that God and the campers' parents have entrusted us to select 'Christ-like' individuals to mentor their children. The main criteria for being a camp volunteer are that you are: 1) a committed Christian, 2) a person who understands the serious responsibility involved in working with children and young people, and 3) a person who is active in the life of their church and involved in your church's youth group or children's ministry programs.
I agree to review and edit the content of all my social networking sites so that any language, photos, conversations or other content is appropriate and consistent with a Christ-Centered life. I understand that camp Directors may look at my sites prior to my acceptance as a counselor for camp and agree to confirm them as friends for this purpose. I understand that this is important in being a good example to younger students who will be "friending" me on, and viewing my social networking sites after camp.
The information contained in this application is correct to the best of my knowledge. I authorize any references or churches listed in this application to provide any evidence or information (including opinions) that they may have regarding my character and fitness for working with children or high school students. In consideration of the receipt and evaluation of this application (by the directors of the SDCF's Mt. Palomar Bible Camp), I hereby release any individual, church, youth organization, employer, reference, or any other person or organization, including record custodians, both collectively and individually, from any and all liability for damages of whatever kind or nature which may at any time result to me, my heirs, or family, on account of compliance or any attempt to comply, with this authorization. I waive any right that I may have to inspect any information provided about me by any person or organization identified by me in this application.
I further state that I have carefully read the foregoing release and know the contents thereof and I sign this release as my own free act. I have read and understand that this is a legally binding agreement. I further understand that a background check will be conducted on me, and I consent to any such check.

This Express Assumption of Risk, Complete Waiver and Agreement Not to Sue and Indemnity Agreement, is made by ____________________ ("Camper or Volunteer") if Camper/Volunteer is 18 years of age or older for himself, his agents, partners, associates, successors, heirs, assigns, representatives, attorneys, and all persons acting by, through, under, or in concert with Camper/Volunteer (and is made by the undersigned parent or legal guardian ("Parent(s)") of Camper/Volunteer for and on behalf of Camper/Volunteer if Camper/Volunteer is not 18 years of age or older) in favor of San Diego Christian Foundation, Inc., its past and present members, volunteers, agents, partners, associates, affiliates, directors, officers, employees, predecessors, successors, heirs, assigns, representatives, attorneys, and all persons acting by, through, under, or in concert with it (collectively, "SDCF").
Camper/Volunteer wishes to (and if under 18 years of age also the Parent(s) authorize and grant permission to allow Camper/Volunteer to) participate in the Mt. Palomar Bible Camp ("Bible Camp") to be provided by SDCF.
In consideration for being accepted by SDCF for participation in Bible Camp to be provided by SDCF, the undersigned Camper/Volunteer and Parent(s):
Camper/Volunteer and Parents understand that SDCF from time to time produces promotional material relating to its programs and that Camper/Volunteer may be included in videotapes or photographs taken during Bible Camp so, without reservation or limitations, hereby assign, transfer and grant to SDCF, its successors and assigns, licensees, sponsors and all other commercial exhibitors the exclusive right to photograph and/or videotape Camper/Volunteer and to utilize such videotapes and photographs and Camper's/Volunteers name, face, likeness, voice and appearance as part of Bible Camp, in advertising and promoting similar future events.
I understand that SDCF is relying on this Agreement to accept Camper/Volunteer for participation in Bible Camp and would not accept Camper/Volunteer in Bible Camp if Camper/Volunteer did not execute this Agreement.
We have read and understand this Agreement. We understand that by signing this Agreement, we surrender valuable rights. We do so freely and voluntarily. We further certify that we have received and read a copy of this Agreement.
